Office Policy
This page contains useful
information that you need to know before visiting our office,
please read carefully to ensure that you are well prepared for
your appointment.
We are participating
physicians in most commercial insurances including Medicare.
Please contact our office Insurance Coordinator, Lisa for
more information at 425-453-0766.
Please be
sure to bring complete insurance information with you to your
appointment:

Insurance card. If no
insurance card is available, please have ID#, group #,
billing address & phone # of insurance company

If a referral form is
needed, and we do not have it at the time of your
appointment, you will be responsible for the bill.
It is your responsibility
to know the requirements of your particular medical plan.
For instance:

Do you have a co-payment
for doctor visits? (If so, this is due at the time of your
appointment.).
We do
not accept credit or debit cards. Cash and check only.

Are you required to have
prior authorization from your Primary Care Physician (PCP)?
If you are not sure, please call your insurance company. We
are not able to do this for you.
