First Visit Information
We recommend that you write down any issues important to you before your visit so we can assist you in prioritizing them. We endeavor to treat issues important to you in a caring and comprehensive fashion, but we may suggest that you book an additional appointment if more time is needed. Thank you for your understanding.
Office Policy
Please read carefully to ensure that you are well prepared for your appointment.
We are participating physicians in most commercial insurances including Medicare.
Please be sure to bring following information with you to your appointment.

Insurance card. If no insurance card is available, please have your ID #, group #, billing address & phone # of insurance company.

Referral form. This is needed at the time of your appointment and if we do not have it at that time you will be responsible for the bill.
It is your responsibility to know the requirements of your particular medical plan.
For instance:

Do you have a co-payment for doctor visits? If so, this is due at the time of your appointment. We do not accept credit or debit cards. Cash and check only.

Are you required to have prior authorization from your Primary Care Physician (PCP)? If you are not sure, please call your insurance company as we are not able to do this for you.

FAQ’s
What is a copay?
A co-pay is the portion of your healthcare visit fee that you are responsible for paying at the time of check-in. The amount is dictated by your insurance company and it is considered a portion of the cost for your office visit. You can check the front of your insurance card or call your insurance company for more information. Our clinic will charge a processing fee in addition to the co-pay amount if the co-pay is not paid at the time of the visit, so we recommend that you come prepared.
What if you can’t make the appointment?
Please give us at least 24 hours notice so that we can offer your appointment slot to someone else. Some patients ask to be put on a waiting list and your consideration in letting us know that you can’t come in will help other patients who are anxious to be seen. You will also avoid the late cancellation fee ($25.00 for regular appointments; $75 for full physicals).
What about parking?
We are pleased to offer free, above ground parking for your convenience.

We are located directly across from Overlake Hospital in Bellevue on 116th Avenue North East.
What about access for the disabled?
Our clinic is fully ADA accessible and are pleased to have a level ground entrance for all patients.
